Instructions
- After logging in, click on “My Activities” from the left navigation menu.
- From the drop-down menu, click on “Document Replacement”.
- Click on the “New Document Replacement” button.
- Select a reason for document replacement.
- If the document was lost or stolen, indicate the police report number. If you do not have a police report number, please provide a brief explanation.
- Select the program for which you need the replacement document.
- If the reason for the document replacement is a Name Change, you will need to submit supporting documentation.
- Check the box next to the document(s) you need to replace.
- When requesting to replace a Welder Logbook, you will need to submit a passport style photo of yourself.
- Provide your mailing address for the delivery of the replacement document.
- Review the details you've entered and click on the Submit button to proceed with your request.
- If your replacement request requires a fee, you will be directed to the Checkout page to proceed with payment.
- Review and confirm that you have read the Terms and Conditions before clicking on “Pay Now”.
- Upon completing a secure payment, your document replacement request will be successfully submitted. Track the status of your request from the My Documents Replacement section.
