Instructions

  1. After logging in, click on “My Activities” from the left navigation menu.
  2. From the drop-down menu, click on “Document Replacement”.
  3. Click on the “New Document Replacement” button.
  4. Select a reason for document replacement.
  5. If the document was lost or stolen, indicate the police report number. If you do not have a police report number, please provide a brief explanation.
  6. Select the program for which you need the replacement document.
  7. If the reason for the document replacement is a Name Change, you will need to submit supporting documentation.
  8. Check the box next to the document(s) you need to replace.
  9. When requesting to replace a Welder Logbook, you will need to submit a passport style photo of yourself.
  10. Provide your mailing address for the delivery of the replacement document.
  11. Review the details you've entered and click on the Submit button to proceed with your request.
  12. If your replacement request requires a fee, you will be directed to the Checkout page to proceed with payment.
  13. Review and confirm that you have read the Terms and Conditions before clicking on “Pay Now”.
  14. Upon completing a secure payment, your document replacement request will be successfully submitted. Track the status of your request from the My Documents Replacement section.
